BLI Program Overview, Selection Criteria, Program Dates and Program Fees

To be considered for the BLI Class of 2021, please complete the application below and submit the required attachments by March 8, 2021.  The BLI Committee will select candidates in April, at which time the Committee will contact all candidates as to whether they were selected to the BLI.  If you have questions about the application or requirements, please contact Shannon Reid at


  • Prepare the required attachments before filling out this form.  Those attachments include: (1) one letter of recommendation addressed to BLI Committee Chair R. Lee Robertson, Jr. (PDF); (2) the completed commitment signatures form (click here to download the form); (3) your resume (PDF); and (4) a professional headshot (JPEG).  Note: If you are interested in an updated photo, please contact Greg Hicks at to schedule an appointment, and please indicate to Greg that this is for your BLI application.
  • Type out your longer responses in a separate document first.  When you are ready to submit the application, copy and paste those responses into the electronic form (e.g., responses to the Vision and Perspective questions).  You cannot save the application midway through.
  • Do not refresh or close out of the window.  After hitting "submit" at the bottom of the form, wait until you receive a confirmation message on screen (you'll have to scroll to the top) before closing out of the window, and do not click to refresh the page content unless you wish to clear the screen and your responses.
  • Record a self-introduction.  Once your application is submitted, please introduce yourself to the BLI Committee by submitting a video that is 60-90 seconds in length that tells the Committee who you are, where you work and what you do, and briefly, why you want to participate in the program.  Click here to email that video to BLI Facilitator Dave Newell and MCB Associate Executive Director Shannon R.
